➧ Step 2: Submit application to competent authority and wait for approval. 

Depending on which health insurance category you belong to, the place to receive your application will be different, specifically:

• Subjects participating in family health insurance: Submit application to health insurance collection agent;
• Subjects registering for health insurance through the enterprise: Contact the HR department directly;
• Retirees, children, people with revolutionary contributions, social security beneficiaries, and people with disabilities: Submit application to the Social Security agency of the district where permanent residence is registered.

➧ Step 3: Receive health insurance card (*) with new information.

Within 7 working days from the date of receiving complete and valid documents, the Social Insurance agency will reissue a new health insurance card to you.

While waiting for the card to be changed, you will still enjoy the benefits of participating in health insurance.

2. Procedures for changing the initial medical examination and treatment registration place online 

The method of changing the initial medical examination and treatment place online on the Public Service Portal only applies to individuals. The steps are as follows.

➧ Step 1: Access the Vietnam Social Security Public Service Portal, then log in to your personal social security account and select “Declare profile”.

➧ Step 2: Find and select procedure code 612b “Re-issue, change health insurance card due to change of time for 5 consecutive years, initial medical examination and treatment place”.

➧ Step 3: In section (17), fill in the information of the medical facility you want to change and upload the initial set that fdiinvietnam.com instructed in the section above, then enter the confirmation code to complete the procedure.

Note:

Employees who are participating in health insurance at the enterprise should contact the HR department directly to change the initial medical examination and treatment place at procedure code 613 (for the enterprise’s social insurance account).
