I. Benefits of participating in social insurance
Each individual’s social insurance book is the basis for the social insurance agency to resolve benefits for social insurance participants (workers). Including:
- Receive maternity benefits;
- Receive occupational accident and disease benefits;
- Receive pension when reaching retirement age;
- Receive social insurance at one time;
- Death benefits for relatives…
In fact, losing the social insurance book does not change the employee’s social insurance participation process. However, that individual will not be able to complete procedures to enjoy social insurance benefits such as: health insurance, social insurance, unemployment insurance and other regimes mentioned above.
II. Cases of re-issuance of social insurance books
So what should you do if you lose your social insurance book? Can you reissue a lost social insurance book? Or can you reissue a lost social insurance leaflet? The answer is, according to Article 46 of Decision 505/QD-BHXH, cases of reissuance of social insurance books include:
- Re-issue social insurance books (cover and separate sheets) in the following cases:
- Damaged or lost social insurance book;
- Merge social insurance books;
- Change social insurance number;
- Change of last name, first name, middle name of social insurance participants;
- Change date of birth of social insurance participant;
- People who have received one-time social insurance benefits but have not yet received the benefits due to unemployment insurance contributions.
- Reissue separate social insurance book in the following cases: damaged or lost social insurance book.
III. Documents and procedures for re-issuing social insurance books
1. Re-issue lost or damaged social insurance book (no information change)
1.1. Submit application for re-issuance of social insurance book via post office
The procedure for reissuing a social insurance book without changing information when lost or damaged by submitting documents by post is as follows:
➤ Step 1: Prepare documents to re-issue social insurance book
Application for re-issuance of social insurance book includes the following 2 types of documents:
- Social insurance book re-issuance receipt form – code 607a (for individuals);
- Declaration form for adjusting social insurance information (form TK1-TS).

➤ Step 2: Submit application for re-issuance of social insurance book
- Agency receiving documents:
- Working people: Submit application to the Social Insurance agency where they are participating;
- Voluntary social insurance participants: Go to the collection agent or the social insurance agency to collect directly;
- Retired employees who are reserving their social insurance payment period: Submit application to the social insurance agency at their permanent or temporary residence.
- Social insurance agency processing time:
- In case the application is complete and valid: Within 10 working days from the date of receiving the application, the Social Insurance agency will process and reissue the Social Insurance book to the individual;
- In case of having to verify the social insurance payment process of individuals in different provinces and different units: Within 45 working days from the date of receiving the application (with written notice to the employee).
1.2. Submit application for reissuance of social insurance book online on VssID
Case 1: Through VssID software applied to individuals
The process of submitting an application for reissuance of a social insurance book online when lost or damaged via the VssID application includes the following steps:

➤ Step 2: Select public service
➤ Step 3: Select procedure 607A to reissue social insurance book without changing information
➤ Step 4: Fill in the information to receive results via postal service and send
Case 2: Through the Vietnam Social Security Public Service Portal
The procedure for reissuing a lost or damaged social insurance book for individuals submitting applications at the Public Service Portal (Vietnam Social Insurance Electronic Information Portal) is as follows:
➤ Step 1: Access the link https://dichvucong.baohiemxahoi.gov.vn/
➤ Step 2: Select to log in to your personal account
➤ Step 3: Select procedure 607a to reissue the book without changing the information
➤ Step 4: Enter the address to receive the attached file, enter the verification code → Click confirm
2. Re-issuance of social insurance book due to change of information (full name, insurance book number…)
Applicable to cases where the insured person (employee) needs to change the book number, last name – middle name, date of birth, change the ID card number to CCCD, the person who has received one-time social insurance but has not yet received the unemployment insurance payment period.
2.1. Submit application for re-issuance of social insurance book by post
➤ Step 1: Prepare documents to re-issue social insurance book
In case individuals participating in social insurance submit their own application
Application for re-issuance of social insurance book with change of book information includes the following documents:
- Social insurance re-issuance application receipt – code 608;
- Declaration form for adjusting social insurance information (form TK1-TS);
- Other relevant documents:
- Birth certificate or birth certificate extract;
- ID card/passport
In case an individual (employee) submits the application through the unit
Application for reissuance of social insurance book for employees with changed book information includes the following documents:
- Social insurance re-issuance application receipt – code 608;
- Declaration form for adjusting social insurance information (form TK1-TS);
- List of information on social insurance participants (form D01-TS).

Note:
- In case an individual submits the application through the workplace, the unit is responsible for confirming the TK1-TS declaration, registering the adjustment of the employee’s information, and is also legally responsible for the content that has been confirmed, signed and sealed;
- Regarding the information table (Form D01-TS) above, currently in Ho Chi Minh City, the insurance agency no longer requires additional declarations in the file. However, in some other provinces and cities, this table is still required.
➤ Step 2: Submit application for re-issuance of social insurance book
- Where to submit application:
- Working people: Submit application through the unit where they are working or submit to the Social Insurance agency where they are participating;
- Retired employees who are reserving social insurance payment period: submit documents to social insurance agencies nationwide.
- Time for processing application: Social Security agency shall adjust the content of the social security book within no more than 5 days from the date of receiving complete application for re-issuance of social security book.
2.2. Submit application for reissuance of social insurance book online on VssID
Case 1: Through VssID software applied to individuals
The case of re-issuing a social insurance book due to information change online via the VssID application only applies to individual applicants.

➤ Step 2: Select public service
➤ Step 3: Select procedure 608A to reissue social insurance book due to information change
➤ Step 4: Declare the information that needs to be changed and click submit
Case 2: Through the Public Service Portal of Vietnam Social Security
For individual applicants
➤ Step 1: Access https://dichvucong.baohiemxahoi.gov.vn/
➤ Step 2: Select individual, log in to personal account
➤ Step 3: Select procedure 608a to reissue social insurance book due to information change
➤ Step 4: Fill in the address to receive results via postal service
➤ Step 5: Declare the information that needs to be changed in section II and attach related documents → Confirm to send the documents
For applicants who are organizations or units where individuals work
➤ Step 1: Access https://dichvucong.baohiemxahoi.gov.vn/
➤ Step 2: Select organization, log in to organization account
➤ Step 3: Select procedure 608 to reissue social insurance book due to information change
➤ Step 4: Choose workers
➤ Step 5: Select the employee whose information needs to be changed
➤ Step 6: Declare information that needs to be changed into TK1-TS
➤ Step 7: Declare information on the attached document list in D01-TS
➤ Step 8: Attach documents
➤ Step 9: Save to save the profile, declare to send the profile and export the form to render the pdf form
