What is a social insurance leaflet?

The social insurance leaflet is an indispensable part, attached to the social insurance book, recording the employee’s social insurance payment process. The social insurance leaflet includes: annual leaflet and book closing leaflet.

➤ Annual flyer:

  • Issued after each year the employee pays social insurance and unemployment insurance (UI);
  • The annual leaflet is the basis for confirming the process of participating in social insurance and unemployment insurance contributions fully paid by the employer as of December 31 of the fiscal year.

➤ Closing sheet:

  • Is the basis for determining and resolving social insurance regimes;
  • Issued to employees who have not participated in social insurance for a full year (from the beginning of the year to the time of leaving the company) or in case of moving outside the province.

In short, the social insurance leaflet is an important document to fully determine the employee’s social insurance participation process. This is also a necessary condition for employees to enjoy some regimes such as one-time social insurance and unemployment benefits according to regulations.

If I lose or damage my social insurance leaflet, can I get a new one?

Yes. When you lose your social insurance leaflet or the leaflet is damaged for any reason, you just need to apply for a new social insurance leaflet and prepare the documents according to the regulations. 

Cases where separate sheets are reissued are specified in Article 46 of Decision 595/QD-BHXH including:

  • Lost or damaged leaflet; 
  • Merge social insurance books together;
  • Change social insurance number and important information such as: last name, first name, middle name or date of birth; 
  • People who have received social insurance benefits once but have not received enough social insurance contributions.

Instructions for procedures to request re-issuance of social insurance leaflets (download free form)

1. Request for reissuance of social insurance leaflet directly at the social insurance agency

➤ Insurance participants need to prepare documents, including: 

  • Declaration of participation and adjustment of social insurance and health insurance information (Form TK1-TS);
  • Certified copy of CCCD/ID card of the insured person.

➤ Submit application for reissuance of social insurance leaflet:

  • For employees participating in social insurance at the company, submit the application to the company or directly to the social insurance agency;
  • For employees who are reserving their social insurance payment period or have received pension or social insurance benefits, submit their application to social insurance agencies nationwide.

➤ Wait for the Social Security agency to respond and return the results:

  • Within 10 working days from the date of receipt of valid documents, for normal cases;
  • Or up to 45 working days in some special cases such as: employees paying social insurance in many different provinces/cities, many different enterprises…

2. Request for re-issuance of Social Insurance card online on VssID

In the age of technology, social insurance participants can carry out the procedure to re-issue social insurance leaflets online on the VssID application simply with the following 3 steps:

➤ Step 1: 

  • Log in to the VssID application with your social insurance number and password issued by the social insurance agency. Refer to the article: Instructions for registering for VssID;
  • On the “Personal Management” page, select “Public Services”.

 

➤ Step 2: 

  • Select “Reissue social insurance book without changing information”;

Note: In case you need to update information, for example, changing from ID card to CCCD on Social Insurance, select “Re-issue Social Insurance book due to information change” to request a re-issue of the Social Insurance leaflet with the latest and most accurate information. 

  • Then select the address to receive the results as “Via postal service” and fill in the information according to the instructions.

 

➤ Step 3: Click “Submit” and wait for results.
